Direct Ship Return Form

Fill out this form only to return an item that was shipped directly from the vendor.

Any item picked up must be returned to the store.

Product(s) must be returned within 30 days from the date of purchase.

Product(s) should be unused, in the original packaging, and in resaleable condition to be accepted.

A copy of the invoice of sale or the package slip accompanies the returned product(s).

The customer is responsible for arranging the return and all return shipping costs.

Warranty: As a distributor of goods manufactured by others, warranties are not provided for products supplied. To obtain the warranty information, consult the manufacturer’s website.
